All you need to do is go to gravatar.com and create an account (with the email address you use when you post comments on the site). From there you can add a image from your computer or the web. It is fairly easy to set up.

The only confusing part is that the menu to login and upload your image is kinda hidden in the upper left hand corner.

Click on manage, you should be able to select the one you want to use there.

I think they check images, so it may take a little while for it to be approved.
